Homeowner Waivers, Liability Release Forms & Rebuilding Day Checklists

Information for Homeowners Brochure (Revised 02/08/05) - Tri fold brochure given to homeowners, which briefly explains our mission, what to expect at various stages of the process, and how to prepare for Rebuilding Day. The contact name, house captain or project manager, and telephone number fields may be completed on your computer before printing the brochure -- click in the space provided to enter your information.
House Captain & Skill Captain Handbook (Revised 03/09/07) - This handbook contains information about the Rebuilding Together Baltimore program and it explains the responsibilities of House Captains & Skill Captains. It also contains all of the forms & checklists listed below.
- Volunteer's Agreement and Release from Liability - This form must be signed by each volunteer, and presented to the folks at headquarters before participants may work on a project.
- Parental Consent and Liability Release for a Minor - Children as young as 14 are allowed to volunteer and work on Rebuilding Day. There is work for every skill level and those students who participate may be eligible to earn some of their community service hours.
- Homeowner's Agreement and Release from Liability - Every homeowner needs to sign this form before any work can begin on their property.
- Homeowner's Waiver Form - At the end of the day, when all of the work is finished and the house is clean, the homeowners are asked to inspect the work and sign this form.
- Scope Of Work & Project Summary - This must be completed by all House Captains. It briefly describes the project and the estimated cost of repairs.

- Volunteer Roster - Use this form to list the names, phone numbers and special skills for each volunteer on your work team.
